I suppose it depends on intention, first, and utilization, second. If a filmmaker thinks that a particular image needs to be very present, and utilizes that as so, it's not heavy handed. However, that doesn't mean that it's clever. For it to be clever, it needs to have an element of invisibility to it. If it's difficult to catch, perhaps hiding in plain sight, that's clever.
