OP pic pretty much did it for me. I was bawling. I think it's because it seems so sudden thank to the "twist," which is the way death really feels. Someone you love is there and then BAM! they're gone. I actually think that's the most functional and least gratuitous twist I've seen because the reveal has a big impact.
Also:
The end of "Blackadder Goes Forth" from Hugh Laurie's "I'm scared, sir," through Darling's wish to marry Doris and all the way up to the fields of flowers and birdsong. Such a short period of time but fucking devastating and twists the knife harder in a few minutes than whole series or movies manage.

when I was a kid, I watched a cartoon show where a teacher of one of the characters was planning to retire, then it seemed like she might decide not to retire, then all of a sudden at the end of the episode someone comes in to announce that she'd dead. Later I learned the voice actress had died in real life which is why the character died in the show. It was horribly traumatic. Can anyone remember the name of the show or which episode this was, or was this some awful childhood nightmare? I think it was a main character's little brother whose teacher died. It was awful.

In Naoki Urasawa's Monster at one point the antagonist takes an orphan boy to the red-light district to show him where he was born. The orphan witnesses a woman being raped in an alley and almost kills himself when he realizes that his life was created without any form of meaning beyond adult desires. Nothing in a tv show has hit me more than that I think.

Series 5 seemed less real to me. It had some punches but after Series 4, it was hard to see where they could gone that would have been worse.

Is it odd that I didn't feel any sadness from the scene in OP's pic? Scrubs is usually more deliberate about telling you what's going on through JD's narration, and for better or for worse I'd gotten used to that so when the twist was revealed I was honestly more confused than sad. I did feel a little bit near the end but really, the scene where Cox lost three patients because of his mistake made me feel more than "Where do you think we are?" did.
