They aren't there completely in the pregnant Rose model sheet. It's highly likely a shadow.
Also if they were markings, you'd think they'd play some part in her fusion with Pearl. But in Rainbow Quartz they don't show up in any form.

I prefer her Gem being in her mouth, on her tongue or in her throat. It it's one of her eyes, it throws any possible fusions off balance visually. So far the Crew has been careful to place all Gems central on the body, excluding Ruby and Sapphire who will probably never fuse individually with anyone else.
