Users will sometimes display obsessive behaviour over fine details and feel inspired to begin on impossible projects.
Long term use risks include: brain damage, nervous tissue damage, shaking and loss of controll of limbs, inability to digest food, contracted blood vessels, and a blue tongue.

If cooked properly has more mild flavor and makes the taste of things it's mixed with more vivid, because of this it has a decent value and is popular as a food among the middle class that can't afford great ingredients.

You know could laugh at it but look up star jelly. Call it Night weed and it'll be a strange unatached leafed plant just sorta appears overnight for no reason. Smoked it causes a lethargic satisfied state with minor mostly auditory halucinations when abused. It spread rapidly at first with people just picking it up until the mob got invovled and patrols the open fields it seems to appear in during the early morning. Now that people are hooked and the various crime guilds are trying to push it it's causeing some problems.

Though what is this Night weed? Clearly invisible creatures sheading layers off them self as they fly through the void. It only looks plantlike it's just strange alien leather. The parts make it to earth and will actual drive people mad/mutate them if they abuse this shit for years or when they inevitably use alchemy to concentrate it.

While it doesn't actually increase the strength of it's users, it makes them less caring about bodily injury due to over-exertion.

A drug that allows you to astral project to other planes. It's a real out of this world experience, man.
In my game there's a plant that gives one use of the spell Wish, although it's owned by the government of the lizard people, and stealing it is not only hard as fuck but would incur some of their worst punishments, because obviously you don't want random people just wishing shit, or growing their own wish plants. However, this has already long ago affected their water supply, making the generations upon generations of lizard people who drink this water more magically adept and casting Prestidigitation on anyone who drinks it.

Balor's liquor (Noble)
Ingested (liquid)

A spirit brewed with the blood of a holy person. It bears some dangers as a mundane poison (though mostly in large doses), but is consumed for its remarkable ability to induce a sense of euphoria and spiritual wholeness. The holier the person, the greater the impact.

Balor's liquor provides an inherent danger because the satisfaction of ordinary good works and ethical standards is still not comparable to its effects; its distribution often (though not necessarily) compels the nobles who consume it to abuse their power to commission as much of it as possible, ironically pursuing a sense of enlightenment through evil.

Dragon bones.

In the past, there were creatures (primoridal gods) that were so large, so powerful, they had multiple souls. There was no concept of heaven and hell then, so when they died, their souls entered their bones instead.

If you smoke those bones, you can inhale those released spirits, giving you supernatural insight and strength but opening you up to possession. The street name for the drug is dank souls.

To expand this mechanic. Chugging health potions a lot causes health potion tolerance that takes a week to reset by mechanics, but actually lore wise it could take year(s) since it's subjective. Chugging them daily for more than week causes addiction. Symptoms are unexplained pains, bleeding,fatigue/weakness, general malaise and easy bruising. They're instantly cured with taking another health potion. Addiction gets worse depending how long you use them, but obviously for mechanical reason there should be some cap at some point.

Health potion overdose is also a possibility. In this case user feels very euphoric, but sleepy. Eventually (if overdose is severe enough) consciousness is and user dies of respiratory depression. There are rumors that some health potions addicts are so addicted that to feel health potion euphoria they need to consume deadly overdose.

An magically infused stone that, when ground into powder and ingested raw, makes a person slightly stronger, tougher, and/or faster as well as providing a rough euphoria. This would be used by gangers/bandits/gladiators/etc as a sort of steroid or a hard narcotic. Excessive use would lead up to something along the lines of "Rocklung", where the dust and stone builds up to a point in the user's throat/lungs that they suffocate, potentially to death. If used sparingly, the drug dissolves in the body over time.

The ore can be refined and/or distilled through magical or alchemical (either way expensive) means to be soluble, and then added to an alcohol or other potion to completely avoid Rocklung as well as enhance the effects- both the physical improvements and the mental euphoria. This version of the drug would be accessible only by the wealthy!

It's called that for a reason. It comes in ~2mm shards of yellow-white, glassy matter, derived from sea salt traded out of a port town ruled by a living voodoo pirate god. Stored amusingly in socketed jaws of ivory skulls. Real skulls if you're a badass or a drug lord.

You can't chew it or you'll OD on the spot, it feels like literal chunks of broken glass to swallow, you'll cough blood for the next half hour, and a bad high might result in becoming a mindless beast person for the next fifteen hours. But fuck it'll get you high. And the biggest draw is that it completely numbs your body from any kind of pain response. Not only do you not feel pain- or anything for that matter- but your body will continue to function as if nothing has even happened regardless of the (non-fatal or completely immobilizing) injury. Basically gives anyone something like a Die Hard feat.

It's ideal for keeping the peasant workers from going into cardiac arrest after 72 hour shifts, fueling rabid drug-addled gladiators, and for the noble folk: unwinding after a long day of political bullshit, as long as you don't try to pull your own skin off.

Zombie Powder. Mummies and liches give the highest grades, but anyone can enjoy pain relief, spiritual numbness, reduced lifesigns, and a wonderful floating feeling as well as a slight vulnerability to Command Undead.
Overdose causes necrotic lesions in the digestive tract or nasal passages.
Though it's not addictive, it is VERY useful, and also absurdly cheap to make as long as you can raise the dead. The usual operation is a schmuck with a wand and a few trays of rats, which are zombified, crushed flat, and left to dry.

Funny this reminds me of campaign I once ran where I tried steering the party toward the main plot but then they promptly gave up after discovering a rare brand of weed smoked by Dragonborn which was many times more potent thn regular 'pipe weed' (they called it 'Dragon Kush') and proceeded with the money from their adventuring to set up the 'weedery' where they grew the stuff and retired as rich succesful merchants.

anyway a drug from the same setting:

Avera:
A thick and highly addictive black potion which can be made by most alchemists with relative ease although the quality varies hugely depending on the ingredient quality and the method of production.
It can be sipped neat, put into annother beverage or boiled and one inhales the vapours. It simply induces a sence of great euphoria quite simmilar to heroin. The side effects however can be quite severe, over time it simply numbs your ability to experience any kind of emotion, forcing you to consume ever higher dosages of it to experience the same high.
One who consumes too much simply becomes an emotionless zombie, unble to feel anything, usualy ending up self torturing or killing themselves or others in order to try and experience any kind of emotion. As such it is obviously highly illegal in most places.

Dragon Jaw Ash

Dragons have a membrenous lining in their maws that protects the soft palletes from the effects of their various breath abilities although this is most common in species of Draconis that utilize strong acids.

After a while a new layer grows underneath to replenish the protective membrane which causes the dragon to lap at the flaking remnents with it's tongue and spit it out due to it's disgusting taste.

Typically, Jaw Ash is used as a component in powerful explosives and destructive spells to enhance their effects but others have found such uses for them as grounding up the ash into a fine powerder mixing it in with sweet smelling flowers in some cases and snorting the Ash which causes rapid heart beat and an energetic sense of euphoria with some describing it as having a dragon's fire inside of them.

It's been observed to have some strength enhancing qualities as well with some warriors snorting dragon jaw Ash before battle and fighting on in spite of otherwise damaging wounds. However, once the high wears off the user becomes supremely depressed and all supression of pain is apparently amplified as the nerves are still sensitive from the previous high.

Prolonged usage of Dragon Jaw Ash results in lost of motor functionality (ie.e excessive shaking of hands and limbs, unable to grasp things) muscular dystrophy, suicidal thoughts and tnedencies and death usually from injuries sustained while high.

I've actually had this idea for a while!

I call it Wizard's Elixir due to the fact that its made out of the bones, organs, blood, and skin of dead wizards, all ground up into a slurry. The inherent magic inside it makes it taste like the imbiber's favorite drink every time, along with giving those who never learned how to use magic a temporary ability to cast randomly selected spells or cantrips. If one is already a wizard, this provides a temporary boost (+5) to their Int, Wis, and Cha. The side effect is that its incredibly addictive, and also has a high chance of driving continuous user's insane, i.e,
on the off chance someone gets wish, or gate from drinking it, there might be a pretty big problem.

We're not thinking magical enough here guys... this is D&D. I could see "drugs" being downright silly or childish. Like those jellybeans in Harry Potter that can taste like anything be it deep dish pizza or rotten eggs and it's random.

Or perhaps a drug that boosts mental awareness and memory (int and wis bonus) but makes a magical copy of yourself that is a dumb twat and prone to getting himself in danger.

The kicker is the real you is actually the dumb clone, the smart you is the illusion that's trying to protect your high as fuck ass. Typically if the imaginary you figures out they are the one that's imaginary the drug trip immediately ends.

A candy-like drug that causes euphoria, impaired balance, and creates an illusion that the user is in a psychedelic maze. The other symptoms wear off, but the user is left in a stupor and unable to wake up until he escapes. Abused by petty thrill-seekers, the drug has no long-lasting side effects, other than that the maze seems to get larger and more intricate every time you take it.
