And the Timmy the Tarrasque comes back.

So the Lady flays him an infinite number of times, and finally manages to expel him from Sigil.

The Lady realizes she's getting old. Not old enough to give up the ghost (so to speak) but old. So she takes some well needed R and R somewhere else.

Ensuing power vacuum caused by one of the most powerful beings in existence leaving is horrendous and bloody.

But fuck it, because the Lady is really loving vacation.

Some schmuck decides to contract the PCs to bring her back to Sigil, because he's tired of plane-wide gang wars.

Getting down at the club, PCs find her incredibly out of her mind on drugs, and maybe end up falling under the influence, too. Ensuing "What did we do last night?!" scenario.

1) I don't know about 2nd edition, but in 3e and 4e, animals are True Neutral, and in 5e, they're incapable of having an alignment. The Lady of Pain might not even be capable of giving a shit about animals, let alone be offended enough by one to flay or maze it.

Even if the Lady bothers wasting 2 lvl 9 spell slots to destroy him, it would be quite a bit easier to cast Banishment the 2-5 attempts it needs before it takes.
