I'd hardly call the Sword pseudo-doom. It's much more of a hard rock/stoner metal setup. Age of Winters, Apocryphon, and High Country are all fantastic albums which exemplefy their skill. Hardly uninspiring.

Wayne Reynolds happened, mostly. 3.x exploded because it sort of rode this hunger for "realistic" adventure, and representations of adventurers loaded down with bags of gear and spiky angles. Pic isn't on-topic but shows the stark differences between older art.
He wasn't the only one; Wizards also brought in artists like Brom (of Planescape fame). In hindsight you can see there was a very conscious attempt to distance the art of the new editions from everything that came before, for better or for worse.
I also think the whole OGL thing played into it too - it opened the door for lots of third party publishing and thus less centralization of the defining look of a fantasy RPG.
Plus at the same time you also had the maturation of World of Warcraft as the sort of de facto fantasy video game representative, with huge pauldrons and exaggerated proportions that fit right in with the 3.x aesthetic (Warcraft certainly was around for longer, but not really as an RPG representative). Contrast the Warcraft look with earlier Gold Box games, or heck even within the same company Diablo I and II look very different.

I don't know how I'd describe 2000s shit other than "comic book" sort of look. Doesn't look real at all, but also doesn't look fantastical. Worst of both worlds. Classic stuff often was both.
Here's classic Brom, who is actually better known for his Dark Sun covers, of which this is one. The devs came up with shit based on the art he sent in. These guys use their bell as a psychic focus to attack travelers in their sleep, then sneak in to their camp and devour their essence (good ol AD&D2e permanent stat drain).

It's the artists and developers working on 3.x who were the most vocal about diversity. Marketing and sales at WoTC understood their core demographic to be white males, so they wanted a typically white male and forced the inclusion of the Human Fighter Regdar into the lineup of iconics. The artists responded by mostly depicting him in really bad situations - most Regdar art shows him dead or dying; he has the fewest heroic moments of all the iconics.
There's no SJW boogeymen here, just artists with a particular agenda going against marketing.

There's also some truth to the "diversity agenda" claim, although again like the link in >>47584957 points out this was a conscious decision by the developers rather than pressure by marketing (if anything, the marketing and sales people wanted the opposite).

I don't think there's anything wrong with having a burly black fighter or middle eastern wizard, but combined with all the other baggage 3.x artists were reacting to (characters with grecian anatomies, real-world armour and clothing, scantily clad ladies, lavish narrative backgrounds, basically everything posted in this thread) this resulted in a sort of hideous mishmash without any real lasting power.
3.x's Mialee is a perfect example of this - a contrarian reaction to all the typical elf stereotypes that just ends up looking hideous. You can almost see the thought process going on: "our elves need to be really alien and should seem unearthly and not conform to human standards of beauty," an attempt at gritty and realistic fantasy that doesn't conform to visual stereotyping - a laudable attitude, but one that sort of undermines a lot of the folkloric structure that fantasy draws it's foundation from.
