Don't ask me, I've been working on a CYOA for "months" that someone else either lost motivation for or got too caught up in life to finish. They even posted a WIP of it, too.

The... choices being relevant? It's not the subject matter, that just attracts attention. A CYOA can be good regardless of its subject (yes even "insert thing you don't like here" could be goodbut only if the person making it was good enough to do so in the first place) It's not the visual design, that just increases the likelihood that people will remember it. The content has to be both balanced and have an impact. There has to be a logical reason for someone to choose any of the options presented, not just a few that could be considered "the best". If you can present the choice as something that could be useful or at least alleviate the pain of a bad situation (See: Island CYOAs) then it's a good choice if you ask me. AND IT SHOULD FIT THE THEME OF THE CYOA, CHRIST.

Hit the high notes, hit the low notes, and imply things. Just look at Quiet CYOA. Implying things goes a long way as far as lore is concerned because dry histories are just that, dry. Besides, if you leave room for interpretation instead of saying "this is exactly what happened" it keeps people interested in the hows and whys of the setting.

I want to make a full weeaboo cyoa, a japanese urban fantasy. You'd be a member of a Clan of magicians, demon-worshippers or something, and would have to prove your worth to be chosen as the successor by the current Clan leader. I've also got an idea for a system where runes affect each other but it's looking like this is going to lead to having a lot more symbols than I'd originally planned for or a fairly complicated system where you conjugate the runes differently based on context. The whole thing is built around the grammar of constructing an enchantment, since runes are language-based magic, so I'm actually pretty excited about the latter option but I feel like I'm just tacking a lot of mechanical cruft on for my own enjoyment. I dunno, I should work on that prototype more today.

The weakness of the male option really becomes obvious with the goddesses. The male benefiting quests barely exist, and have zero real benefit to them. If you're a woman, you can take a quest to become queen or conquer male cities.
