There's no strict definition of fantasy creatures, perhaps unfortunately. You can't honestly say that kobolds are only minorly draconic small lizard dudes who are shit, as much as I might prefer it if that were the case. Other established backgrounds are in use.
Building off that, as far as I know D&D is the only system that I know for sure makes as big a difference between orcs and half orcs as it does. Other systems/settings just have Orcs and Humans. Elder Scrolls and Warcraft for two very popular video game examples, though those ar on the orkier side of things.

I don't understand what the problem is.
The orcs didn't evolve a way around the mammary glands or anything. They simply never had those in any part of their lineage, because they were born from the element of fire via magic during the prehistoric age. Even if they had evolved naturally, they aren't mammals in the first place. Also, the fact that they even reproduce sexually is a failsafe that appeared after the original method of reproduction for the four elementally-aligned races, spawning of children from elemental keystones, was destroyed by the dragons in an ancient war.
The dwarves also lack mammary glands, as they are a single-gendered race that incubates new individuals within a stone-like growth on their spine. Elven females only have breasts because the Fae altered their physical forms to be attractive to humans, who are the most numerous race.
