I'm trying to create a Trait(chosen at character creation)in my tabletop RPG that simulates Insanity. Any ideas on what type of mechanic it should be? It should have upsides and downsides.
If it matter, it's a GURPS type RP

Personally, I can't stand sociopathy PR psychopathy anymore. And that "superpowered evil side" has been done to hell. Try making someone with down syndrome or a deadpan snarker with some kind of autism and ADD.

Have your characters look up the mental disorder of their choosing and study it. Then you give them XP as reward for good roleplaying.

Character's sex doesn't match their gender - they insist on being called the opposite pronoun, will dress in inappropriate clothes, etc.
i had an insanity racial trait in my 5e homebrew
the characters could go into a state of madness which doubled their proficiency in certain skills but they had to roll on a chart that made them do ridiculous things.
I suppose I should have mentioned that it's a Superhero/Villain RP. So the Insanity should be analogous to Joker, Creeper, etc

Apparently there's a correlation between some psychological disorders and increased intelligence, but I don't remember where I heard it so I'm pretty much pulling information from my ass.
Has anyone heard anything about that?

Insanity doesn't make you intelligent.
But intelligence can give you psychological problems, most commonly a depression.
