I'd say he's very different, everyone has an Id or Shadow (and yes I acknowledge these are not at all synonyms) but we do what we can to keep it under control. For most people, I wouldn't think it'd be a difficult choice at all to kill yonder supervillains, but for Bats it would break the dividing line between them and us. I don't think Bats would be gruesomely horrified to know a cop killed a criminal (unless it was one he turned in), but I think that's symbolically his breaking point that would fill him with deep shame and a feeling of lowliness, in the sense of a personal taboo rather than actually believing the Joker's life has great value.
The Heat Legend Joker points out that Bats isn't very different from him, which is true, but he also points out that ordinary people aren't very different from him, either. I would absolutely say Bats is more like a normal person than he is like the Joker, morally or psychologically. He deals with his hangups in a bizarre fashion but its more productive than usual.

In no way is it Batman's job to kill people. If it would be Objectively Morally Superior (TM) to kill, say, the Joker, then Joe Schmoe the cop could do it, and say it was self defense. Or the state could in general do that job in an intentional fashion. He turns his incredible talent, skill, and artifice to help Gotham and public figures, in most franchises, are grateful for his help. Hell, the Bat signal is emblematic of it, and literally so (it has an emblem of him).
I don't see any reason to think Batman is paranoid. Compare Batman's actions to, say, a number of governments, which at any given time has contingency plans to cause WW3 and nuclear armageddon.
