Weird thought: One of Kabuto's signature senjutsu moves is the White Rage Technique. It sends out constant pulses of chakra to blind and shock opponents, sort of a ninjutsu flashbang. In this continuity, he was probably inspired to make it by Kon's bloodline. It is more or less a bootleg version of what we can do with our Shattershots.

You prepare yourself for battle as Kabuto dives toward you in his new form, carefully watching the snake connected to him, as it moves independently from the rest of him. Don't want to dodge only to get caught off guard.
You're ready to counterattack, but Kabuto stops just short of you and forms some hand seals before placing his hands on the floor of the cave.
"Sage Art: Inorganic Reanimation!"
Suddenly, the cave's many stalagmites and stalactites move and extend in your direction. You jump to dodge, but they follow you, new ones joining in wherever you jump. The movements are far more complex then you've seen any Earth jutsu. It's like the cave itself is fighting you.
"This jutsu is different than controlling an object with your chakra." Kabuto explains. "It temporarily imbues an object with life, using my chakra. They can then attack you of their own accord. In a cave like this, there's no way you can dodge for long."
Kabuto's right. Despite your enhanced agility, the spires of rock get closer and closer. You're being attacked from all sides, and a counterattack in any single direction would leave you open. You're just about to be struck, when a huge red figure surrounds you and blocks the attacks. Looking to your right, you see the Hokage standing next to you. This must be his Susanoo.
"Kon. I need... your help to get him in my Sharingan technique." the Sixth pants. "I need to get an opportunity... to repeat an action against him. I apologize, but you will likely need to protect me... for a time."
Itachi takes a deep breath, then turns from you to look at the spears of earth around the two of you.
"Amaterasu!"
Black flames consume the earthen spires, and the cave begins to warp back to a more natural state. Itachi shuts off his flames and drops his Susanoo, his breath heavy and labored once more. You see Kabuto dive behind some of the stalagmites in the cave's distance.

Hopefully it will protect us from the vibration effects of the White Rage Technique, we'll still be blinded but at least we'll be able to maneuver using our chakra sensing. It would be weird if it didn't. Combo attacks are where it's at, when it comes to dealing with Ten-Tails hosts. No single attack is going to be strong enough to match them on its own. I propose a Kongoshu and Amaterasu Arrow blended together into one attack, with Kiba in his giant wolf form (with possibly as many as nine heads, if Kakashi's nin-dogs have been taught the technique like Akamaru has) warped in right after it by Kakashi's Kamui to hit with a Fang Over Fang.

You're going to need to be better protected for this fight. Kabuto seems to have all manner of attacks, and you should leave yourself vulnerable to as few of them as possible.
You stand alert for a while, gathering the chakra you need to don your chakra cloak and enter Impact Sage Mode. No sooner than you do, do you see something fired from deeper in the cave. It's not aimed at either of you, however. You glance around you to see that they are in fact, spider webs. Kabuto seems to have used them to creature obstructions to your maneuvering in the cave. You get the feeling that they're somehow familiar. Likely the very same jutsu that Kidomaru used when you fought him years ago. Who knows what all abilities Kabuto may have added to himself.
As you get ready to respond in kind, Itachi starts to stumble forward.

Itachi said he needs to do some action if he wants to perform his sharingan technique. As concerned as you are about his condition, you can't just hold him back if you want this to work. You'll have to support him from here.
You knock down a hole bunch of pillars near the Hokage just in case, keeping a close eye as he makes his approach. You're able to catch Kabuto as he makes a dive for Itachi.
"Impact Release: Shattershot!"
Kabuto is struck, but instead of getting knocked back, he seems to collapse into a puddle.
"Don't think I don't remember what your release can do, Kon." Kabuto tells you. "With this ability I got from Suigetsu, I can liquiefy my body and protect it from vibrations. It likely wouldn't help me if you got your hands right on me, but I of course know better than to let that happen."
As Kabuto jumps back again, you can see what looks like Kimimaro emerging from his stomach.
"Braken Dance!"
At Kabuto's word, bones rise from the floor of the cave, threatening to impale the Hokage. You blast them to the best of your ability, but the nature of the attack makes it difficult to stop. The Hokage is forced to jump back to your position to avoid it. With the opening, Kabuto once again makes his way to cover.
You can see the color draining from Itachi's face by the minute.

So, uh, not the most polite thing but I feel it needs to be said: If Itachi does die here, I want to take his eyes. It doesn't take that long to transplant a Sharingan, we could have them swapped in and adapted to them in time for the next stage of the fight. We need the power up, and Itachi is the sort of person who would say some "don't waste this" or "carry on my legacy" thing so he'd probably request us to do it himself if he lasted long enough to get out those last words.

Depends on if he knows senjutsu. Only nature chakra can harm Obito when he's in his giant chakrazord mode. Which means that if Oozaru can't use it Sasuke's going to have to fight him alone. He's the only one who has the combo of senjutsu and giant chakrazord. Naruto has the chakrazord, and might have senjutsu too (despite there being no one to teach him that, but there was no way for him to learn Nine Tails Chakra Mode either and he has that, so...) but he's unlikely to have pulled his heel-face turn at that point since it's Madara he's against, not Obito. Our best bet would be boosting Sasuke's strength.

As long as Kabuto has things to hide behind, every approach is going to go the same way. You need to give him no room for cover. Though you are worried about the Hokage. His condition looks to be getting worse and worse.

While Itachi downs one more pill, you take to blasting away every rock formation you can see. It takes a while, but eventually the entire cave is clear. You see Kabuto standing, preparing a jutsu.
"A wise move, depriving me of a way to keep out of your line of sight." Kabuto says. "But I have other methods."

A white dragon emerges from Kabuto's mouth, carrying a black orb. It travels a distance, then explodes in a painful flash of light and sound.
You can't see or hear, and the pain is distracting. It takes a few moments for you to adjust to it, but you can still use your Sage senses. Kabuto is moving towards you, seemingly unimpeded by his jutsu.

You can move around like this, but the Hokage likely can't. You need to make sure he stays safe. You hurry and get in front of him, ready to respond to any attacks Kabuto sends your way.
As Kabuto gets close, you raise a barrier in front of you, and expel Impact chakra, hoping to push him back. You can sense Kabuto hit the barrier, knocking him backward for a time. It seems to have stunned him, as the light and sound wear off before he tries another attack. Itachi manages to recover as well.
"Kon, I think the best way to do this is to let him attack me." the Hokage tells you. "If he strikes at me with a more basic attack, it's easier for me to replicate. I just need you to keep him from getting too far away, or from using any of his larger jutsu. We need to keep pressure on him for this to work, he's too flexible if we let him be."
You and the Hokage begin to close in on Kabuto.

If that's the case, then you'll have to force Kabuto to attack Itachi up close. You focus and start erecting barriers around Kabuto, slowly limiting his range of movement. By the time he notices he's almost entirely blocked off, Itachi is right up on him, kunai drawn. The snake emerging from Kabuto's stomach quickly lunges at the Hokage, but the look in Itachi's eye tells you that that's what he wanted all along. The Hokage explodes into crows upon being hit, his signature substitution. He reappears close to you.
"Halfway done." the Sixth pants.
Before the two of you can mount another attack, Kabuto has gone on the offensive.
"Demonic Illusion: Phantom Sound Chains!"
A woman emerges from Kabuto's stomach with a flute, and plays a tune. Suddenly, you can feel everything start to warp around you. This must be genjutsu. You try to break free, to no avail. However, you catch a glimpse of Itachi's eye.
"Tsukuyomi!"
The Hokage's illusion overrides Kabuto's, setting you free.

You could attack Kabuto, but Itachi needs to finish the steps for his jutsu. You quickly dash toward him before Kabuto can attack, and cut him slightly, enough to jolt him free of the genjutsu.
As Itachi regains his senses, Kabuto sends out several snakes in various directions, ready to attack from a variety of angles. You get to sniping them, as Itachi summons his Susanoo and throws some kind of projectiles. He throws one at the snake coming from Kabuto's stomach, but Kabuto jumps up and lands on the ceiling. This is where you take your chance. You form another barrier, boxing him in. The only way out is straight down. As Kabuto falls, Itachi draws up to him to attack once more. As Kabuto's snake lunges, Itachi tells him something.
"And this is where you trap yourself, Kabuto." the Hokage says, before he bursts into crows again. "Izanami!"
As Itachi reforms a bit away, Kabuto lands and slumps his head down. He doesn't move anymore.

The battle seemingly over, you drop your chakra cloak and run over to the Hokage.

But the best you can do is heal his wounds from battle. The damage caused by his condition is not possible for you to treat. He's exerted himself far too hard, and overdosing his medication didn't help. His lungs are tearing themselves apart.
"Hang in there, Lord Itachi!" you encourage him, trying your best to get him stable. "You're not dying here!"
"You're right... Kon." Itachi groans. You can hear the slight gurgle of some blood in his throat. "I can't afford... to die here. I have a village to protect... You don't have time to deal with him, so you ignore him as long as he isn't hostile. Dosu walks up to Kabuto.

Dosu walks off in another direction after a bit, but you don't see where, you're too occupied. You're not sure you could move the Hokage and keep him alive long enough to get to Tsunade. Then, you hear something.

You glance over to see a snake emerge from Anko's neck, and from the snake's mouth emerges a familiar man.

Snake Nerd is written as unbeatable without Itachi massively weakening himself, Dosu shows up and revives Orochimaru without any opportunity to even wonder "hey what is this asshole doing here", let alone interacting with him, and now it's either Itachi dies getting him to Tsunade or Orochimaru possesses him. And probably runs off with his body while calling us a faggot with zero opportunity to interfere with him, either because we "didn't realize what was happening" or were "too tired from the chakra expenditure".
