How do you make an adventuring dungeon realistic or at least plausible.

A tomb with treasures is probably the closest thing found irl, go look at how they did it with the pyramids. Traps and shit.

Why is there this place underground? Was it a castle's old dungeons? An ancient city's sewers? An old, abandoned mine? Dwarves? Lava-tube? Is it an old, haunted manor instead? Did some rich, crazy asshole decide he wanted a secret underground ruin? An ancient underground city?

Frankly, why are people just going off and building dungeons, populating them with monsters and traps, and then fucking off for a thousand years?

the best dungeon designs I've seen played through are ones where there's a logic - ie, a guard chamber, intoa hall way, with a latrine off to the side of the chamber. you dont want youur guards going off on long trips to take a piss.

chambers lead to and from, in logical paths, not mazes, etc.

do that,and you get a logical, internal consistency to the design.

I mean, just look at maps of real life sewer systems, mines, bunkers and caves. It's not exactly rocket science.

If you want ruins, check regular building ground plans and keep in mind that they were not meant to be buried underground. Just learn more about architecture in general. For example:
