To be true, I find Tieflings silly. There shouldn't be an entire race with demonic heritage, and it also implies that only humanoids can have a demonic background. What if I want to play a half-demon crab man.
I wish that 5e had tieflings be mongrels by default and confirm that Asmodean tieflings are the vast majority in SCAG. If they'd simply ported the varying appearance and stat stuff to the PHB and kept the lore in both regarding heritage in all settings and the way in which Asmodeus claimed FR tieflings, it'd be perfect. There might actually be art besides the same exact style that it's been since 4e.

Tieflings only have a tidbit of fiend blood. They are often born to two human parents.
Half demons are usually divided into Alu-fiends (female) and Cambions (male) and can have many different appearances. They are also much stronger than normal PC races and would have steep level adjustment.

Planescape artwork does feature some weirder tieflings, but they're still mostly some combination of horns, tail and/or goat legs. Probably because those are traits that immediately say "fiendish ancestry" from a glance, while a tiefling that looks mostly normal but has red eyes and smells like sulphur wouldn't be immediately recognizeable as such.

I generally don't like Tieflings, much like the Drows they are mostly presented as "lol evil" or unique special snowflakes who are also mostly dickbags. They're a separate race in the same sense hlf-elves and -orcs are, ie. mostly in game terms. Half elves are what you call humans with elf blood (or elves with human blood), while tieflings are humans (or other races, but usually humans) with fiend blood. They're not really a separate race (barring 4th edition which made them the descendants of a human kingdom that had their bloodline cursed with fiendish traits), but mechanically are different enough from baseline humans that they're treated as one.
Also unlike with drow playing a non-evil tiefling isn't really particularly snoflakey, since it's not like they have a culture built around being chaotic evil assholes. They just are more often than not assholes because they got bullied as kids or whatever (and maybe the fiendish traits affect their behaviour to some degree, making them more agressive or whatever).

Nah, that's still how it works, at least in 5th edition (4th edition tieflings were different, and they still exist in 5th alongside the "classic" tieflings. An offspring of a fiend and a human is a cambion (if male) or alu-fiend (if female). A tiefling is at least two generations removed from their fiendish ancestor (so one of their parents could have been a cambion/alu-fiend), sometimes considerably more. So you can have two normal looking people and their child ends up being a tiefling because one of their great-great-great-great granparents fucked a succubus and they still have a small amount of fiendish blood that for some reason ended up "activating" and manifesting on the child.
4th edition tieflings were effectively their own race, though. They were still technically human, but were descendants of an empire that worshipped devils and due to some pact or curse all developed a devil-like appearace. Their children also have the same appearance, although I'm not sure what would happen if they have a child with a regular human. I'd presume the child ends up being a tiefling or else they'ir fiendish traits probably would've diluted out of existence since their empire collapsed a long time ago.

Sigil has portals to practically eveyr imagineable plane somewhere, and denizens of Prime Material, all the Outer Planes and more regularly come face to face with eachother. Would you say I'm on the right track with that outline? That would be the initial story, and then there would probably be a few stories that were mostly slice of life interspersed with adventures around Sigil, and then later I could introduce the tiefling's (great?) grandmother (the succubus) as an antagonist.
The angle I was trying to build with the tiefling and painter's relationship was that she uses him as an outlet for her people-eating desires, and he finds inspiration for his artwork from the strange way she treats him (protective but also callous, degrading but also nurturing). As they fall in love he learns to appreciate the fact he can placate her urges (even if he still finds being eaten somewhat scary and gross), while she realizes that she likes protecting him from Sigil‘s many dangers with her physical strength.
Also, suggestions for ways two young not-quite-adventurers can get in trouble in Sigil would be appreciated.

Sounds good. How exactly the characters met isn't something I'd consider particularly important as the main focus should be character interaction, and the plot could be anything that results in the two falling for each other. The main focus should probably be on their relationship developing (would probably take some time for the tiefling girl to accept she really loves the guy, and longer for her to be willing to admit it to him). I do like the idea of the tiefling's succubus grandmother coming in as an antagonist at some point. Maybe not outright villain, but she'd probably try to convince her granddaughter to act more like her (and also try to seduce/eat the guy). That seems like it'd result in pretty good material for plotlines.
Sigil's a great place in that pretty much any possible scenario that one can think of can be justified with it being Sigil.

Tieflings are awesome when they actually look demonic and monstrous, not just anime elves with tiny horns, yeah.
Personally I love half human and half demon characters in fantasy games, especially when they aren't always just 'misunderstood' and actually have dark personalities or impulses because of their heritage.
However I'm not a fan of tieflings in general because, as mirrored by opinions in this thread, they are too strange and mutant to really fit in to human society. Humans are pretty xenophobic and tribal by nature, not that there is anything wrong with that, but a baby born with horns and a tail won't be accepted.
Instead I like making people born to demons and their direct descendants more like pic related. They are totally human, but tend to be pale, have dark hair, and are a bit creepy and weird.
They also get subtle and weak magical powers, often starting as children but sometimes in puberty. Such as an imaginary friend who is actually real and can interact with the world as a weak spirit, or the ability to create false images in reflective surfaces, or the power to mimic voices or the sounds of animals so well its basically supernatural, or you can point your finger at someone and they can't take another step towards you.
This is a much more subtle and mystical alternative to random demon-mutant creatures.

Is it just me or do Tieflings get more hate than even Drow these days? In-game as well. I mean, I can kinda understand it in later editions, but most of the games from 3.x/PF era and back I've been in that include Drow the GMs totally dilute the societal backlash. I play a female Tiefling Warlock girl with infectious optimism. She's always dreamt of being a big hero and wasn't about to let something as small as having fiendish blood stop her.
Charlatan background with False Identify feature, expert in using disguise kit and forgery kit, casts Thaumaturge and Minor Illusion like they're going out of fashion.
She pretends to be a wizard and claim that her Book of Shadow is her spell book. Uses both physical disguise as well as magic to appear to be human. Bluffs everyone into thinking that she comes from an order of wizard called "Tower of the Eternal Flame" and claim that her EB and Hex spells are result of forbidden magical research (aka Magic Initiate feat).Deep down though she suspect making a pact with fiend is going to come back to bite her in the ass. Until then she wants to do as much good as she can.
