Well, that's entire up to you. The reason so many Old Ones were able to be taken advantage of is because they had undeveloped minds or were made weak. Rom was literally without thought or understanding, blocking the Moon Presence and Amygdala and all that goofery from spilling into the waking world. Mergo was an infant, something the School of Mensis wanted to exploit to gain higher understanding. Ebrietas was 'left behind' by her fellow Great Ones and was similarly exploited by the Choir and the Healing Church. In fact, the Kos Parasite looks a lot like an infant Great One itself. For all we know, the phantasms in her body might be more of her children, stolen and experimented on.
The difference with you is that you aren't stunted or weak. You know the follies and crimes of those around you, you've witnessed all the deception and lies and horror in this mad grasp for power made by Byrgenwerth and its spawn and even more than that, you've grown to rival the Great Ones in insight by consuming the umbilical cords. I figured she was just another part of the Hunter's Dream, albeit a sentient part. She's clearly made in the image of Gehrman's doll which is itself made in the image of Maria. I figured it was given life by the Moon Presence as part of the Hunter's Dream it created for Gehrman.
The only sticking point is the fact that the Hunter's Dream remains even after the Moon Presence is defeated. However, there was another Great One that could have taken over maintaining the Hunter's Dream after the Moon Presence died.

The doll is certainly a creation of Gehrman, modelled after Maria, so saying she's "a Great Old One" might not be entirely accurate because that would imply she got three thirds or something. It seems more like her "essence" is mostly a dream-thing which vaguely influences her presence in the waking world, through making the doll fingers move a little.
It might be better to say that she is a vessel for the Moon Presence, who was filled with its power when Gehrman accepted his role as MP's surrogate child. A construct within a construct, so to speak.
The similarities between her and the Winter Lanterns suggests this Doll might not be the only doll to have ever existed. Perhaps this is just the one that Moon Presence got right. On the other hand, the appearance of the Winter Lanterns seems like they might suffer from an over-abundance of insight - all those eyes and brain-stuff. Perhaps the Winter Lanterns are Maria, not the Doll, with Maria's quest for mad insight in Old Hunters echoing the WL's appearance. Note the swollen heads she creates and the swollen heads of the Winter Lanterns.

One theory I heard is that the entire town of Yahar'Gul was sacrificed in an attempt to bring back an Old One's dead child, and the result was the one reborn.
The theory was that the town was flooded with a special blood that was meant to convert the populace into a new body for the child, but instead transported everyone it touched to the exact same spot and telefragged each other into a horrible franken undead thing.
The reasoning for this are the statues clinging to the walls can be seen as people fleeing something in the street as well as the others which seem to be praying or cowering, which signify fear at impending death. Another part of this reasoning is that pool of blood that works a a teleporter.

No, I always interpreted the Moon Presence as being one of the main sources of the beastly scourge (which is why it moves like a beast, why it can 'manipulate blood' with that attack that turns all your health temporary) so it predates Gehrman. I also get the idea more that MP is the parent in the relationship and Gehrman is the child.
I think Gehrman might have used an umbilical cord on the Doll in an attempt to bring her to life after he lost/distanced himself from Maria but how it would have worked on something inorganic I don't know.

Gehrman also sleeps better when you kill the Orphan, because he's no longer trapped in the Fishing Village dream. He's punished for his sin of killing Kos, who was alive when she washed up on the shore and killed when the Hunters raided (according to the strategy guide).
They also fight similarly, using a curved weapon in a fast and highly aggressive manner involving lots of leaps and bounds.
Orphan is what Gerhman would "really" look like if we had eyes to see, a surrogate infant to an inhuman beast more Kin than man.

Ebrietas was called the "daughter of the cosmos". We find out later, as it was mentioned in this thread, that Micolash had a kind of revelation in "Kos, or, some say Kosm?" where Kos' name was meant to be Cosmos.
Not sure if it's just a thematical linking, but the connection intrigued me. Maybe Ebrietas was the daughter of Kos, herself?
