He uses his sword as a magic brush, but to do so he needs to see the world in 2D as if he was painting on a canvas. Two eyes would ruin the illusion. He writes magic runes in the air or something to cast spells. There was a 4E Swordmage paragon path about writing runes in the air with a sword now I think about it.

Philosophical reasons.
His motto is "An eye for an eye makes the whole world blind".
His eyepatch reminds him every day never to use his sword to avenge.
In a world where military and martial might are the measure of a man, indeed the only real power, violent revenge is the obvious answer to any slight. His father died on a quest to avenge his grandfather, who himself had died on his own quest of vengeance. He's seen too many close friends and kin die either on quests for revenge, or as the victims of vengeance themselves.
He knows that none of them were happy as they died, and he knows nobody was really happy in achieving vengeance. It only leaves behind more vengeance, more suffering, more hate. Another generation of pain.
So he'll suffer any price to avoid that savage cycle of revenge. No matter how deserved or sweet it would be, no matter how painful or humiliating the offense, he'll bear it. Because he knows can't bear the alternative.

Different kind of character, but I once had a mage with a cursed eye that let him see the future. The only problem was that he saw ALL possible futures at once in the eye and not anything always useful. It's disorienting and confusing so he keeps it covered up.

A sentient, parasitic worm ate half of his brain and is coiled up in the empty section of his skull. The man begrudgingly allows the worm to stay since it controls most of his bodily functions (digestion, heartbeat, etc). The eyepatch covers his eye hole, which is the worm's breathing hole, and also muffles the creature's numerous snarky comments. Occasionally, the worm emerges to feed on the flesh of dead enemies.
The "in over his head" trope is always fun: he's impersonating another (dead or living) famous fighter who lost an eye. If the former, maybe he eventually comes into his own and takes over the legend; if the latter, maybe he has to confront the person whose identity he stole?
