What are game design or even adventure design concepts that used to be around but aren't anymore? We have a lot of people into OSR type games, but even those games use modern concepts with a slight twist on them. What actual design elements have we outright lost and never use anymore?

Bad ones usually. There's a reason THAC0 is dead and I'd wager the same applies to any other extinct mechanic.

Maybe dismemberment rules. A lot of the games I've played lately haven't had ways to cripple an enemy and run away, they were just walking life bars that don't go down until they reach zero hit points.

Mixed XP requirements for different classes is another D&Dism nobody wants to resurrect. Hell even giving different amounts of XP to different characters is falling out of vogue.

Different XP Tables were what slowed down the Wizard rising to power.

At 2500xp:
The Wizard just attained level 2, getting 1d4 hit points and an extra daily use of a 1st level spell, for a grand total of 2 spells per day (3 if specialist).
At the same time the Rogue is now level 3, with 3d6 hit points and dealing extra 2d6 from sneak attack.

But I now think the whole XP is old. New games should use CR for encounter making and awarding a new level at key points. XP is an archaic mechanic from the old times where sandbox play where a thing.

XP and CR are both artifacts of wargaming. RPGs as a whole are moving away from that and into narrativist or simulationist directions. The idea of an encounter being balanced is pretty silly in both fiction and any sort of simulation with competent strategists. A good story has reasons for one side being much stronger (though no necessarily winning) and a good general makes sure they never get into a fight they might lose.

Yeah, but CR is for the GM making the encounters know easily how it will go and how easily he can adjust it to make it powerful or weaker than the PC party.

Design systems in world. Not a point buy meta thing, but putting components together to reach game world mechanics.

King balance (which can never happen) putting the optimization in the meta level and not in the game world.
