My edgiest twist is still being set up, and it's that one woman helped put in motion every major issue in the entire campaign.

There was a necromancer mentioned in the campaign but not yet met that has been screwing with people's lives from afar.
I'm planning on having her show up as an inconspicuous enemy flanking the 'main' villains who plays along because she 'won' thirty years ago and is watching the conflict like cinema now.
It wasn't ALL her fault so much as she was an enabler for things turning out how she wanted- eternal life gets dull, you know?
She watches the parties like a TV show and the villains like a sitcom of evil.

Yeah, and she's not willing to stand her ground against an opponent she can't one-round either.
She has a whole Bag of Holding of voyeur (for her romance kick), recorded adventures (action), and political intrigue (you see where I'm going here).
A dedicated treasure trove of recordings of people she's screwed over for memory playback.
One of the character's family was killed by ghouls, kicking off his quest for eternal life.
Guess who.
Another bad guy got screwed over due to her recommendations and then recruited due to the fact that she played both sides.
She had also conspired to get the same guy kicked out of academia and his homeland, effectively breaking his life at every turn until he was desperate enough to become a villain.
If the party ever tells this guy (they've fought him before, he's likely coming back) and explained how his life was basically permascrewed by this chick, he'd probably lie down and die out of shame or lose his shit and kill everything that moves.
