Why doesn't everyone with enough rank and influence in 40k just get their body replaced with cybernetics, so they never have to worry about age or disease?

You might argue that they wouldn't like it, but personal preference shouldn't stand in the way of doing your duty for the Emperor.

That's why a lot of them do. Rich Inquisitors will often carry around cyber eagles or replace an arm with a bionic + gun or get bionic eyes/laser fingers, etc.

The only people who don't get body mod upgrades are the ones who can't and the ones who believe in the purity of the flesh.

Many of them do, others follow organizations such as the Cult of the Pure Form which rejects augmentations.

Either way, Cybernetic augmentations cannot really save them from aging and death. Even juvenat treatments can only extend it by a couple of hundred years.

Unless you're a Navigator.

Pretty much, the thing is Op they can only produce / maintain so many of them.

With the mech being the technophiles that they are they tend to use the majority of them. With then nobles and high ranking members of imperial institutions, and the military using the rest.

Also if you are going full cyber you will probably need to afford the cost of having a techpriest on your staff to maintain you.

THIS. You can't just slap on a robot arm and expect it to work as well as the original without careful maintenance. Augmetics are some complex tech, so you can't just go full Battle Angel Alita and expect everything to take care of itself. The Mechanicus are the only members of the Imperium with enough skill to maintain them.

Presumably, people of extravagant means would refuse to go completely cyborg, because that would mean the end of life's pleasures. You might live for a thousand more years, but is it worth it if you cannot enjoy the subtle taste of exotic spices, the tones of the most alluring music, or engage in the carnal pleasures of the flesh?

This is why, when they get desperate, people don't turn to the Mechanicus, but instead go down the road of the Ruinous Powers.

I love how in the fluff it often references individuals of moderate rank as being something like, "She was only 80, still a young neophyte and the body of a 20-something" or some shit. I don't think modern humans today can ever really fathom living that long, because it would just totally fuck with our minds and our memory.

To the question, many people simply may not need it, or cannot afford the high-quality ones that look almost indistinguishable from the real thing, and would only be able to buy the clunkier (but still functional) normal level gear, and some would even basically just get servitor parts.

Also, while many people aren't against augmetics, they don't want to give away a large chunk of what makes them human if there isn't any need for it.

Moreso, augmentations are vulnerable to attacks that flesh-and-blood creatures wouldn't be, and can be exploited appropriately.

With enough metal bits, the bionic parts of them could keep functioning partially after death.

And the bionic implants can be easily improved and last more or less forever.
