There is no modern work out equipment, instead the workout rooms are massive and filled with logs to toss and rocks to move along with robotic predators to dodge or fight. You are allowed melee training weapons in the room but no firearms or lazers.

Every ten years a resident is chosen to be part of a little experiment, they're forced to endure starvation, sleep deprivation, thirst, pain and extreme temperatures while being injected with trace amounts of FEV+whatever else the scientists think they should this time+a substance that helps them survive this trial.
After the experiment is concluded they are thrown into the wastes.
The vault lacks any ranged weapons beyond crossbows. Every vault member is put through training for their favored melee weapon though. The Vaults originally were slated to open after a century so the radiation could become not instant-kill levels.

The reason everything seems so random is because the vault was actually set up to be controlled by an A.I. based on the copied and mishmashed brainwaves of the resident. Essentially, an attempt to create a true god, driven by the prayers/desires of it's residents.
This did not go entirely as planned. They got the A.I. but it's always built upon the the minds of the current generation.
So, every generation the vault gets a new controlling intelligence.
