The funniest part is transformations caused by Polymorph other decay based on how far from the base creature you change the subject with turning a man inot lets say a Gargauntian Gold Dragon lasting a couple of rounds and changing someone into a Frost Giant a couple of hours is the wizard is skilled.
Changing someone's gender, because it only changes the base creature one shift, is a permanant effect unless disspelled.

You could add "duration: permanent" with the spell ending "when I hit them with it again".
I would also say making it a "once per day" or otherwise making it difficult to cast (so that the joke doesn't get old as fast and you don't overspend) would be appropriate.
At least that's if you're using the powers system. The Magic book has a sex change spell in it, if I remember correctly. In the Body Control College of magic. It's right next to the harmless decapitation spell.

A world where all of one gender has died out and its is required for beings to be polymorphed in order for species to continue living. Prisoners of war, criminals, and people who want to change become the opposite gender so that the species don't die out.

In PF/3.5, there's Girdle of Masculinity/Feminity and Elixir of Sex Shift.
The former's so powerful, Wish/Miracle has a 50% chance to undo it, while the latter is pretty cheap and easy to spike someone's drink with.
Although if someone's dispelling your spells, you're wizarding wrong.

Dispelling is a legitimate issue when people can throw Disjunctions and AMFs at you. Also the wizard himself may not be around to prevent the dispelling. Any way you can jack up your caster level the NPC mage can too.

At the very least, Wish for one of those two magic items to be created around their waist/in their mouth.
In practice, depends on how specialized you want it. You could probably eke out a PaO that goes up to Instantaneous duration from a 9th level spell, but a simple Humanoid -> Humanoid should be around 6th. If it's completely limited to mechanically nonexistant effects? Maybe 4-5.

Something like this might seem a bit weird, but it'd be interesting if someone seemingly mundane might've been something else before magic intervened, whether it is reincarnation, wish or polymorph.

Oh yeah, I do this a shit ton. I like to subscribe to very mystical ways to handle magic, and this can happen all the time. For example, if you are a Wizard and you turn that bastard who slept with your wife into a pig, even if only for a while, when he turns back to normal his pig traits aren't 100% gone. Maybe he'll always be a little chubby or have a 'squeal' sounding laugh, or he'll always be a bit pinkish. This sort of shit happens all the time because of magic, and every town has to have at least one or two people touched by it, either in a good way like they have some potentially mystical talents or in a more negative way, but in any setting with any amount of magic users running around this is bound to happen.
