How do you describe fey portals in your game?

Inconspicuous and gradual. The idea is that you can wander into fairyland and not realize for a long time. Just describe the landscape slowly growing older, wilder, and weirder as the party goes.

Alternatively, inconspicuous and abrupt. Turn a corner and you're suddenly somewhere that is viscerally not where you were before.

The point is that the actual point of transition itself should not be observably magical. No glowing doorways in reality or shining runes or any of that. Just an abvandoned road in the middle of the forest that might be the remnant of a previous kingdom, or a cave in a hill that may look like a weird cave, but not some obviously magical thing.

Agreed in general sentiment, but the OP asked for portals specifically so I'd have to go with the classic door that opens up in the side of a hill that, when closed, leaves no sign it ever existed.

you went for a walk in the woods, followed this pretty little stream down a gully, then climbed up a fallen tree to one edge and crossed another fallen tree to the far side. you think to yourself how beautiful the trees are, and moss on the branches, and the light in the leaves. you think about the enormity of the forrest, the vast depths, as you walk along through the foliage. eventually you come out into a meadow, and watch the breeze sweep across the grass. Off over the mountains, on the edge of the sky, you see a glint, and wonder what it is as it passes by at a steady pace. You pull out a scrap of parchment and a bit of charcoal and try sketching the vista, but soon give up and just daydream as you wander through the open expanse. you find and pick some blueberries, some are plump, a handfull kinda skimpy and sour, and you collect a small pouch of them. you turn back towards the woods, about a mile off now, and hum a kind of thoughtful tune, one you suppose would sound fine played with trumpets or a flute. you follow your path tramped in the grass right back to where you came out of the woods, then down the hill to the log across the gully.
there you see a man, across the gully, aiming to cross the log. he calls out "good afternoon" and you reply in kind. He's wearing nice but battered traveling clothes, a fine, though rather moth-eaten brocaded coat, and a floppy hat over his long braided hair, his features are fine, and you wonder if he's a nobleman. he smiles and makes to stand aside, so you could cross first, though you'd hoped your pause would send that same message. so you both start and pause again. then you venture that he should proceed, as you find yourself hesitant when crossing the high log and wouldn't want to hold him up. he thanks you for your consideration, strides across, and you offer him some blueberries, of which he accepts a few. he smiles even wider claps you on the back, and gives you a swig from his wineskin.
