Based on your description of California being a "bulwark against China", I would think that would give that region a nice mix of cold war esque red fear of communism and the tension of full war with the other NA remnants.

When you say "the entire population of North America now lives in three locations", do you mean that everyone outside those areas were killed, or that the population of the entire continent immigrated to those locations?

Because my mental image of the situation will change drastically depending on which is the case.

Almost everyone in urban environments died, suburbs did better, rural survived pretty well, all survivors congregated to military fort which then turned their labor to making the mega city while defending the territory. Fast forward and the population rebounded and grew like crazy thanks to technological advancements, population topped over a billion in NA alone, before the second apocalypse event knocked out 70% of them all over again. The mega city wasn't entirely killed, there's a large bulk of survivors fleeing in every direction from the mega city ruins.

It's a militarized city this time. And wasn't originally that overpopulated. They weren't magically killed, it was monsters that went in and slaughtered and used the death to make more monsters in a near exponential army.

Also because people no longer know how to live without central hubs of life, it would have grown anew regardless. Government figured it was better to put everyone behind a wall and arm it to the teeth, rather than try and regulate where people can live.

That said, that's how they designed California, and the myriad refuges in the Caribbean.

Eighty meters tall in a circle around the entire mega city. Passage in and out ended up restricted to only through the gates on the rivers. Massive wind turbines with air scrubbers to deal with the wind pressure. Was never actually used because the horsemen just didn't bother attacking from the outside, they got in and used it to trap the people like rats.

Nah man, Europe regressed back to castle based city states. The "mega corps" are cross city guilds controlling the flow of resources with primary interests in the American trade route, salvaging the abandoned cities, and getting resources from the newly recovered forests. They all went the way of the Romans, made senates and constitutions, but going to war means electing a dictator. And theyre almost always at war.

Man hunt fir the guy that caused the second apocalypse, then sold national secrets (How to make AIs by using a live human brain) to China for extraction which was votched and now hes on the loose.

The Gulf Fleet just integrated the mega city super AI into their aircraft carrier and are propping her up to remove Congresses claim to power since she ran the city for the most part.

Great Lakes Region is recovering particle accelerator data which was supposed to give infinite energy, but its actually the theory for next gen FTL. The AI hat did that research went insane and the data hasnt been decrypted due to the second apocalypse.

The dead are still an omnipresent threat

A new religion is gaining ground, treating the heralds of the apocalypse as the heralds of God.

Undead mostly. Unconfirmed rumors from the immediate aftermath of mutated animals taking on the appearance of North Americas extinct megafauna, and mauling people to death.

Biggest threat was the Four Horsemen who led the armies and made monsters as needed, then abandoned them to their own devices. The second apocalyose was when the horseman Congress was keeping in the basement got loose. He showed up for a public speech before two gunships showed up and poured literal tons of bullets into him to buy time for containment protocol. He responded by turning his flesh into a mega fungus and infecting the city with death. Now its a twisted nightmare realm where once there was a beacon of humanity.
