My worst idea to date as a GM is my planned Mass Effect/Mutant Chronicles cross over setting. It's a loose collection of notes right now and I doubt I'll ever try telling my players about it. The whole thing spawned out of a conversation with a friend after a game of warzone about what would happen if the main antagonistic force(the Dark Soul and it's apostles and legions) of Mutant Chronicles was unleashed by some of the Mass Effect races after first contact with humanity.

Trying to fight a Lasombra Ancilla with a hand help M2 machine gun. I did this by charging into an obscured building, with no visual on the target and without even trying to lay down some suppressing fire. Needless to say I got ashed in one round of combat.

Sort of. I mean they're not always actually dungeons, they're more like alchemist labs, refurbished caves/mines, hidden cultists temples/shrines, and decently intact abandoned forts/castles.
Dungeons might crop up in those but dungeons are rarely the whole thing.
Typically don't even do dungeons.
We did one recently that was part of a "game show for funds" as kind of a spoof thing to hand wave travel expenses since I feel that weighs the game down.
But as far as "You enter the room. Roll perception. You see a monster and some traps" dungeons, those have only come up once as a novelty. It was a fun distraction but we just don't fit them into our game's narrative.
We're more likely to encounter "the wrath of nature" (i.e. quick sand, bogs that make carts useless, animals...

Why don't all Tyranids have as sharp claws as genestealers? The Hive Mind is all about trying to engineer the most bullshit unit possible why not give every single lizardbug claws that render armor useless?
My second question is why only marines in terminator armor teleport? Why not regular marines?
It's resource vs output type situation.
Rending claws ae not present on lesser tyrannic forms as it is more resource intensive to make them as well as the musculature needed to make use of the hardness factor. If hoards are needed quantity is better than quality for the same resource cost. If you need deadly ambush/infiltrators those same.resources can then be put towards the more intensive genestealers.
Regular marines can teleport but it is rare and fickle tech that can only move X number of combatants so it is used for the units with maximum effectiveness/numbers...

Kill the level 1 sacred cow in medieval fantasy tabletop RPGs.
From now on, level 1 means a child or a small animal, most adults with real jobs are level 3 or 4, and the starting level for PCs is 5.
This way, a PC's first level up is not such a huge leap in HP and class features that level 1 enemies immediately become a joke, and an illiterate barbarian who multiclasses into wizard will not get all the benefits of a starting wizard who supposedly studied for years.
This will never die so long as Paizo keeps on having all adventure paths start at 1st level, and as long as Wizards of the Coast does the exact same thing with their own adventures.

Also since characters with the same class can go very different routes these differences are much more apparant after just a few levels, making characters a bit more unique from the start.

Both problems you list don't actually stem from starting at level 1 though, and can be easily fixed by just not playing D&D and its derivatives.

Do you guys have that one friend who always pushes a broken custom card idea and asks you about it?
Currently he keeps sending me this 1cmc Planeswalker with "+1 Add G to your mana pool", and despite how much I disagree, he keeps arguing how it isn't that bad. It's been going on for a while now, and this post is partly to rant, but how do I get him to either make it in quiet or leave me alone about it? I've tried dismissing it like a normal human being but he keeps sending me it.
