It's mostly about money and number of fans. One helps the other and vice-versa, i.e. if you have a lot of money you can buy stars, which will attract fans, which will bring more money to the club and so on.

Back in the days, when sport was mostly an amateur passion, a couple of decades of good management was all that took. Nowadays it takes a big injection of (usually foreign) money and big money usually doesn't have the patience to stay put for decades so it's harder.

No, as long as the fans are there and the team has a solid tradition the club will always be a name.
Pouring money is not enough to be big. For example City were a respectable(not big though) name even before the money, but they will really leave a mark if they win the CL for example.
A team can be big at 10th place, it can be big in 3rd division.

Billionaire owners are the way to relevance now, although the historically big clubs have always been good to great for the majority of their existence.
a big club is a consistently succesful club with a great story and a lot of fans around the world. for example, ajax is not a big club because they've got 10 years of glory maximum.
milan is still a big club, they just got a really shitty present, but they always can change that and its greatness will rise again (just like barcelona, for example)
