Before batting helmets it did. There's even a theory that Gehrig's ALS was caused by being beaned so often.

Fun fact about Gehrig. In 1934 the Yankees had an off day so they scrimmaged against one of their minor league teams. The pitcher for the minor league team had attended Columbia University with Gehrig and didn't like him. Gehrig's first at-bat he hits a home run off him and it infuriates the pitcher. Next time up the pitcher nails Gehrig in the head. Gehrig lost consciousness for over 5 minutes and was rushed to the hospital where he was diagnosed with a concussion and was told not to play for a while. Well we all know his streak didn't end there... the next day the Yanks played against Washington. Gehrig's head was so swollen he had to borrow one of Babe Ruth's hats and even then he had to cut it to get it to fit. Lou Gehrig hit three triples in 4 innings. Then a storm came and the game was called, erased from the record books.
