I assume it's because they're incapable of coming to any sort of agreement between each other.
The smaller FAs resent the big ones and the big ones think they should get preferential treatment in the same way that Europe as a whole does in world football.
The big FAs can't break away because they don't want to be left as a small group of outsiders and the smaller ones probably figure they get a better deal as things stand.

I bet clubs fucking hate national teams. If you look at how American teams handle baseball, the teams basically pressure their best players to stay home and the "national team" manager to not play better players because of the risk of injury. In basketball, the FIBA World Cup is essentially a team for second-tier now that two good players have gotten injured, the only reason the Olympic team still has good players is because of dream team nostalgia.
