Preferably captains/coaches getting a limiter number of challenges per match/half.

If you bring video to football it could not be applied on offsides (except when ref not validating a goal because he whistle after it was scored).
It could be use only on penalty kicks and serious fouls (like if it deserves a red or not).
Football is too rigged/corrupt to see it happening though.

Its the logical step forward that needs to be done to improve the game. I would give the coach 3 challenges (like in handegg) where refs get a video review of the play and then decide. If the challenge was wrong, coach is charged with one of his 3 possible subs (like handegg's timeouts).
Sadly this will never happen, cause it would destroy refball match fixing, and ruin FIFA's business.

give each manager two or three challenges per game. if you're worried about the flow of the game, limit them only to big decisions (like penalties, goals, and red cards). the fourth official walks over to a little monitor to watch replays of the incident, and then tells the main shitcunt what the score is.
Kind of a casual here, but aren't both teams on one sideline? Put a track along the opposite side with cameras perpendicular to the sideline, and either automated or manually-controlled to stay even with the last man. It would make reviews fairly easy for offside calls.
As for how they are officiated, it would make sense to do it like turnovers in football. Anything that's even close is played as a continuation, then at the earliest continuation/change of posession, the review would occur.
Regular camera angles can already be used for goals, enforcing diving, and sideline calls.
Honestly I get the opposition to replay, it's rusty at best when introduces in every sport it's in, but it gets refined over time and starts to improve play. If you have the technology to make the right call without severely changing the way the game is played, there is no excuse not to do so.

protip: video reviews simply displace human error (essentially a constant) away from the football field itself and into the lap of the video reviewer. with regards to offside, fouls and simulation, it's still essentially a referee making a decision in the end. The same amount of incorrect decisions would occur, just for different reasons. No longer would blatant simulation occur, but the implementation of video evidence would force fifa rule makers to bring a greater level of rigor, clarity and specificity to the rule regarding simulation. This creates a more complex rule which opens up various interpretations of the said rule. Different interpretations, due to different referees gives different results and ultimately an identical amount of inconsistency (human error) to what we already have. Sometimes things are just against you, sometimes they are not. That is sport.
