I'm going to be nitpicky only because it's a really good fake so your 90% there.
Unless that is how the original picture is you may have gone for a little too much blur. The tit's seem a fraction more in focus than her face. Only off by maybe 0.2 or so if your using Gaussian Blur.
Learn how to use the 'Blend If' function in layer styles. Create a curves adjustment layer and bring down the brightness a little, set it to blend only where the underlaying image is light and paint that just over the right side of her face to even out the lighting and reduce that glare which does not match the scene lighting.
