You obviously need Photoshop or similiar tools to do this.
Step 1. Search for a photo (Donor) that fits your photo by position, lighting, quality etc. the more similiar they look the better.
Step 2. Crop the area that has the cum on it, copy and paste it to the photo you are editing and position it correctly (you can lower opacity to better position it)

Step 3. Erase the eyes, nose and all of the parts that are easy to erase. (in photo I also used Warp tool to better position the layer)
Step 4. Carefully erase the mouth and the rest of the skin leaving just the cum (I use lower opacity eraser to save time, since you can erase a bit more carelessly)
Step 5. go to Image/adjustments, and mess around with brightness and contrast, colour balances. I personally use a ton of selective colour where you can edit specific colours in a layer at a time (real useful here)
Step 6. Add more layers if you need more cum. That's pretty much all you need to know.

No problem. Its good to start with high quality photos and using a bit more time finding a good donor pic.
Oh yeah Step 7. lower the layer opacity, I usually use 85% but you can use lower or higher. This helps blend it together.
Also you can use the burn tool on the background to add shadows and to make the skin look more wet and the dodge tool to add light/remove shadows.
