Impossible. Wanted to specify no "fancy fabric over areas" but couldn't figure out how to say it. I've even had some where you could clearly see nipple underneath this sort of fabric and I couldn't manage to enhance it.

A lot of it depends on the photo.
If the person is wearing black or white, I often try adjusting the brightness and contrast first to see if something's visible. If not, I adjust levels. For adjusting levels, the two outer arrows should be at the edge of the black "graph", and then adjust the center until you see something you like. (If neither of these work I promptly give up.)
I then adjust exposure if necessary. Darken the naughty bits with the burn tool. My signature thing is I grab their skin color with the eyedropper, paint bucket that color in a layer behind it, and then erase at ~20% opacity around the area - sometimes I avoid the nipple, sometimes I erase right over it. Occasionally I'll use the blur tool to clean up.
One great thing I always forget to try is a high pass filter. Seen some great ones done using that.
