You'd be applying under basis (d) I believe. Meaning emigration from Croatia.
The club thing shouldn't apply to you actually. You'd be covered under članak 11 of the zakon o hrvatskom državljanstvu. Meaning on the basis of descendant of an emigrant. So that's different from the "affinity towards Croatian people" basis which requires clubs.
You need to provide all documents (birth, marriage, immigration + parents domovnica), with certified copies and apostille, and certified translation from English if applicable.
Aside from that, basic language skills which you demonstrate by writing a biography and a personal statement in Croatian. And you must be able to fill out the form in Croatian in front of the consular staff.
Then there is the written test which is actually easy because you can study for it ahead of time. I think you just need 10/15 right on questions drawn from a pool of 99. Can be found here http://narodne-novine.nn.hr/clanci/sluzbeni/2012_10_118_2563.html
