Right now thiers a few ways this could happen. all of them a pretty unlikely though, as ww3 would end everything and all sides know this.
I think the most likely would be some Islamic extremist nuking a mainland European country. Since the EU has, in it's naive stupor, become close with Turkey the ability for these extremist to move through Europe from the middle east has become extremely easy. It is completely feasible for them to simply drive a van containing a massive WMD from Afghanistan to a European country.

NATO is there as a means of shooting down NUKES that's what WW3 is all about and it's all a matter of who can strike first and who can prevent the other side from using their NUKES.
No one is going to agree to not using nukes and if WW3 happens America will try to set up as many ways as possible to take out their enemies nucular capability before using their own.
Neither side will want to start WW3 unless they have some guarantee the other side is stripped of nucular capability and since the Americans are pushing so hard for WW3 to happen I'd say they already have something up their sleeve and want to do things quickly before Russia and China come up with counter measures.
