Regulations passed at the behest of the AMA require that an MD perform tasks that a nurse practitioner could easily handle (e.g. resetting a broken bone).

Of course, advances in technology and treatment would have increased the cost of free market healthcare but nothing has driven cost up like the collusion between what basically boils down to a trade union and the government.
