It's a public health thing and in the US at least they bring in doctors and do it in privacy. It's easier that way and you can get full coverage of the population without inconveniencing everyone. Some parents opt out and have it done privately, but more often than not it just makes you a target for bullying. Seriously, it's 30 seconds and done, like a scoliosis check.
