Bad debt accounts for less than 1% of hospitals operating expenses. The vast bulk is from worthless administration/paper pushing tied to a highly fraudulent setup.

Every once in awhile doctors will volunteer at a city center. Hordes of poor people will line up outside starting at 4 AM. When it's all said and done the doctor returns to his/her practice charging obscene amounts of money to help people e.g. $3000 for a root canal and crown for ONE tooth without insurance.
