Then about half way through iirc Leave took the lead and just built up on it.
BBC called it around the 13 million mark since all the main remain places had been exhausted.

My guess is that older voters were underrepresented in polls, they're not going to show up in online polling, they have very high turnout, and were about 75% for Remain.
