It is observable with a FMRI that trans people exist. All it takes is not enough estrogen/testosterone getting to the brain during pregnancy but the rest of the body getting enough.

Gender roles are more developed versions of human instinct, they're not part of someone's sex, but the cultural norm.
Transgender is no a good term to describe people that want to artificially change their sex, because by your own logic that has nothing to do with asuming gender roles, but changing sex.

The first step is being able to act under whatever gender role you want without being ostracized. For some that's not enough, but our medical knowledge isn't to the point that we can genetically change your sex. There are surgeries and drug therapies available that attempt to mirror true sex change, but as a biomedical engineer I can't help but feel they'll be regarded as barbaric and crude compared to future regenerative medicine / tissue engineering.
But these people don't live in the future. This is the best we can achieve now, and for some, the potential benefits outweigh the complications and side effects. It is a major decision, with severe consequences (good or bad), but people should have autonomy over their own body.

I think gender dysphoria is a mental illness and it's symptoms can, and often are, be treated by gender reassignment surgery. I personally know that with other mental illnesses you can treat symptoms as much as you want, it won't get better until you start treating the source.

Ultimately yes, if they had the choice, they would've selected 'Female' on the character customization screen. However they can't do that, and our current level of technology can't do that.
