The DNC would likely nominate Sanders, since he has the next largest amount of delegates. Sanders would likely win based on sympathy votes and being a better candidate than Trump.

It depends if her death came before the convention. Before the convention---there'd be a lot of pressure to give Sanders the nomination although the party would likely give it to someone who had been a loyal party member for his entire life instead of only for the past year or so (Biden). After the convention---the party would likely have the VP Hillary selected ascend to the role of nominee.
