Russia could overrun the Baltic states in 72 hours as things stand, because their conventional forces are much more powerful than what people give them credit for.
The fear is that Russia thinks the West is trying to destroy it (probably true) and when backed into a corner they'll become aggressive to compensate, which is what they've traditionally done. These exercises ratchet up the tension considerably in that regard. Russia is showing many signs of materially and psychologically preparing itself for war.
In this scenario Russia might calculate that a quick invasion of the Baltic States, before NATO's conventional forces have time to mobilize, could quickly seize the Baltic States and hit certain NATO forces/cities with nuclear weapons. The idea is that the American president isn't going to risk the destruction of Chicago or Washington DC in retaliation for Warsaw, much less Narva or Riga. I doubt the Italians and Portuguese have the appetite for war undone far off country they're barely aware even if the war stays conventional, the Germans certainly don't.
So basically:
- Russia launches an invasion and quickly seized their objectives.
- Conducts "limited" nuclear strikes on NATO military formations or even a city or two, threatens all out nuclear war
- Presents the rest of NATO with a fair accompli, the vast majority of their populations won't risk nuclear annihilation for Estonia or Latvia.
-NATO essentially collapses overnight, Russia makes a deal with Germany in particular
- If the US goes to war it risks full-scale nuclear war, if it doesn't the entire global security framework built up since 1945 collapses overnight as all of America's alliances become worthless.
That's how high the stakes are.
All these exercises look cool and shit but when you are actually part of one its just shit show of which sides command fucks up harder

Just reading I've done over the years. I've been following Russia's military reforms and rearmament program since the mid-2000's.
The best options NATO has is:
- Pre-position enough multinational conventional forces in the Baltic states to deter the Russians, but not so many that it gives Russia the idea/excuse that NATO is preparing an imminent attack on St. Petersburg, which they would use to justify a pre-emptive strike
- Baltic forces hold out much longer than the conventional wisdom dictates. I've heard good things about them and they would certainly be motivated.
- The West comes to some sort of diplomatic deal with Russia and ends sanctions against them, which would significantly reduce the chances of war. Ideally Ukraine would become a neutral buffer state to facilitate this.
- Poland and the Baltic states become part of the nuclear sharing program. This would anger/frighten the Russians, but it would provide deterrence.
