We put the patterns to search for in a list and looped over them rather than hardcoding all the replacements like that.

I think I found out why this happens by reading the tampermonkey docs. The script executes on every domain because of @match *. This is fine, but I think that means it will re-execute everytime the url changes on the page, like when you click on a link that brings you to another part of the page and you see "#moretext" appended to your url. Not a problem, I think. Also the JSON list of names looks good, so 10/10 I trust this script.
