The problem here is multi-fold. There aren't enough doctors because they can get paid better even just in the private sector, let alone in the US which is just south from there. The doctors that are there do not get paid based on the quality of the service provided, and practically can't be fired (see: the tumor'd up surgeon). The coverage from public healthcare itself is quite limited. Etc.
