How did guys march into battle knowing that death is a near certainty? I'm talking about the frontline guys in medieval times. Surely survival instinct kicks in at some point and you turn and run the other way? What was in their minds to hold the line?

Feeding their children back home was probably the biggest incentive if anything. A military job was a guaranteed salary, always paid to you on time and provided relative comfort compared to other low end career paths.
Secondly, any military operates like a cult, controlled input of nutrition and stimulus, back breaking physical labor ie: physical training, and emotional/educational reconditioning. Every branch has a specific type of propaganda. All military personnel have the "hivemind" thing going on.
Thirdly, it depends on the person (s) individually speaking whether they have a fight or flight response...but being shot at, or KNOWING it's kill or be killed is a serious motivation to strike first...and hard to ensure the "enemy" is incapacitated.
