Well, that depends on how you define 'worst'. If you're talking about which electoral system is the least democratic, the theoretically worst possible system would be a reverse direct democracy, where every decision made is the exact opposite of the public will.

As far as electoral systems that actually exist, first-past-the-post can lead to some pretty substantial differences between the popular vote and the seat count, especially in extreme cases (although it does have some practical advantages, like artificially increasing the likelihood of a majority). FPTP is especially undemocratic if electoral districts are either gerrymandered or arbitrary as to their share of the population.

That is a difficult one. Choose your favorite between the following:

US - Party primaries being promoted as some sort of election. This would be counted as early advertisement mostly everywhere else in the world and has the explicit purpose of nullifying the other candidates, effectively enforcing a two party system.

UK - Parliamentary system with victorious take all district voting. This distorts proper representation of the people. For instance, a 49% consistent minority across all districts could have no representation whatsoever in the worst case.

BR - Proportional voting. The vote goes indirectly, summation of all the votes from coalition candidates, to each party and then is proportionally split so as to define how many seats each party will earn. It is possible for a person to be elected without votes if his party earns enough seats. Usually one candidate with many votes brings lots of his peers.

North Koreans can, theoretically, choose not to vote by crossing the name in the ballot and depositing it on another special urn. The issue is with what will happen to them if they do so. Or if they do not show up for that matter.
