Don't know about Africa but East-Asian cultures have a tradition of having additional genders. Not necessarily delusional trannies like in the West but actual categorizations for them, some of them up to 4 genders or more. These other genders would be for example old men that would dress up as women and help raise children because they themselves couldn't have kids. It's more prevalent in East-Asian countries because those races exhibit neoteny features so they have less sexual dimorphism.
