It's obviously a philosophical question. The lion's worst enemy is itself. The upside down lion isn't literal, it's a representative of his inner demons. He's everything he couldn't be. The answer to the question isn't definite. If the lion is strong enough to defeat his demons, then yeah. But if he's not then "the same lion, upside down" wins. It's not necessarily a loss that costs him his life, but perhaps he descends into madness, even addiction (drugs, gambling, lionesses, etc.). There's a lion inside us all.

It's what the lion perceives as the least desirable parts of it's self, and therefore, it is a battle that cannot be won, for even those undesirable aspects contribute to the entity that we know as "the lion".
