I recommend Musicbee for sure. I've used Foobar as well, but it isn't nearly as user friendly and enjoyable to use. It can be "optimized" and modified a lot, however.
As far as organizing in folders, I have my music folder on my PC, then every artist has a folder within the Music folder, and every album has a folder, named "year - album name". Works perfectly.
Musicbrainz Picard + media player of your choice. For OS X I use Swinsian because it's similar to iTunes but more customisable and less bloated. I was using VLC but I just wanted that sweet sweet album art view.
