Dying of cancer is a metaphor for crippling emotional and behavioral issues in his lover due to a fucked up childhood. Him being a hospice worker for her is a metaphor for him being utterly sympathetic and understanding of her psychological problems to the point where he can excuse and look past them even as they take their toll on him and his life. Her death is their breakup where she just disappears with no warning like she constantly threatens to do.
I hope you at least got that Bear was about an unwanted pregnancy and abortion.
