This is something that I do worry about. There are conditions which have caused these people to escape or to latch onto some sort of delusion of themselves.
I recall a transgender person talk about how they think of themselves and their internal monologue as gendered, when a woman, and myself pointed out that we don't think of ourselves in our head as belonging to a particular gender, it's not important for "normal" people, but gender appears to be very important for transgendered people, so I think of it as either some form of psychological coping mechanism, or a congenital defect from birth.
So I think that it is some sort of identity disorder either spurred by trauma or brain chemistry, and for some surgical alteration treats it, but for others they are still suffering.
In any case, you can't treat the mentally-ill as less than human.

It isn't really struggling against blacks so much as it is about growing up among blacks and understanding how they treat each other, white people, and everyone else.
Black people are very real, they don't divide themselves into cliques as much as white people do, for them, being black is the clique, and a lot of them get their sense of identity from that.
When I was eating lunch with black folks I did not see a single white person willing to do so, white people are very fickle and cowardly by comparison and don't really treat blacks as people like themselves.
