It's in my top 5 for sure. It's just incredibly accomplished, it's so dense and multi-layered, with so much detail to unpack. Tom folded an incredible number of things into this, it really rewards repeated listening. It's also viscerally quite gripping, not just a mental mindfuck.
Abrasiveness/loudness plateaus once you get to harsh noise/power electronics/death industrial. Venereology by Merzbow is my reference point for "harshest":

But you can probably find tons of similar albums that are about as harsh. It's easy to get that loud, and impossible to get significantly louder.
Inaccessibility is a completely different story — and it's a lot more subjective. Throbbing Gristle and NON sound good to me, but a lot of opera works just stress me out. I have trouble with a lot of contemporary composers, like Xenakis. Noise that features not crushing slabs of sound is okay, but irregular outbursts of noise is just very unpleasant to listen to for me, it's pretty much the equivalent of jump scares in movies or video games. Free jazz is kinda hard to get into, though I think I'm slowly beginning to "get it", I still don't enjoy it that much.
I would also recommend Diamanda Galás if you're looking for harsh/disturbing/inaccessible music.
