How's a cyoa where you play as a doctor in a hospital sound? Diagnosing weird illnesses, treating magicl creatures, performing prostate exams with the inconvenience of hooves
So, if we were to use a rating system for quests, not for quality but for Lewdness, I'd think it would work something like this:
1 Lewds do not ever occur.
2 Lewds only occur in exceptional circumstances, non-canon chunks of the story. Lewds have no impact on the story.
3 Standard lewdness, lewds occur when players seek them out and it makes logical sense.
4 Players are presented with lewd situations and aided in seeking lewds.
5 The world actively conspires to force the players into lewds.
1: Lost
2: Shooting Stars, Hijack, Boat Quest
3: Dirty Work, Spicy Quest, Curse Quest, Unique Quest, Urban Survival
4: Cutie Clash, Northern Excursion, Alchemy Quest, Spicy Quest,

I can blame this in two factors:
A) The quest accumulated a lot of It Makes Sense In Context really quick. It's just one of those things that happen with stories. But yes, I think It'll Make Sense when you have read it from the start.
B) I intended the players to be as confused as AJ feels, which is a lot.
I intended the atmosphere in this to be strong from the get-go, and I really appreciate when people says they like it.

Too much exposition can ruin immersion, but you handle it fairly well I think. Keep it up.
You might want to give us a bit of lore every now and then, though. I'm not reading it right now and I don't remember how smart your players are, but it's safe to assume they can be very obtuse - like me - and I also remember that I missed an opportunity to ask questions, and personally felt a bit disappointed that I missed that opportunity.
So don't hesitate to prompt us to ask questions about specific things when the time comes.
